rep throat는 늦은 겨울이나 초봄에 잘 발생한다. 리노 바이러스성 감기에서 흔히 볼 수 있는 기침, 코막힘은 S.T.증상에서는 흔치않고 고열, 오한, 두통, 오심, 구토, 복통이 흔히 나타난다. GABHS성 인두통은 고열 및 전신에 점상 출혈성 홍반이 나타남. 이것은 겨드랑이에서도 보인다 . 발적이 없어지면서 피부가 껍질이 벗겨진다.
② 예전부터 GABHS는 6 ~ 12세 인두통 환자에게서 대부분 발견된다고 생각되었으나 최근의 연구는 30~65세 환자에게서도 발견된다고 했다. 그러므로 약사는 성인이 인두통을 호소할때 strep성 인두통을 제외시켜서는 안된다. GABHS에 의한 인두염이 보통 후유증없이 낫지만, 어떤 경우에는 strep throat 의 후유증으로 다음과 같은 심한 후유증을 주기도 한다.
예: ⓐ 인두 후부 농양 ⓑ 편도 주위 농양 ⓒ 화농성 경부 림프절염 ⓓ Rheumatic fever증 (모든 관절,심장,피부,중추 신경,피하조직에 병 증상을 가질 수 있음) ⓔ급성 사구체 신염 (혈뇨, 고혈압, 부종, 신부전 등이 나타남 )
③ GABHS에 의한 인두염으로 의심되는 환자는 의사의 처방전이 필요 시 의사에게 보내 져야 한다.
약물 치료 : Penicillin V 250 mg (1회분)3 ~ 4 회 10 일간 복용.
이 치료법은 급성 rheumatic Fever 발병을 예방하며 비용도 적게 든다. clarithromycin과 azithromycin은 GABHS가 원인균이 아닌 타 원인균성 인두통에 사용되는 항생제임.
Etiologies of Sore Throat
Ⅰ. Infectious Causes :
Infectious causes of pharyngitis include viruses (up to 70% of cases), bacteria (responsible for most of the rest of cases), fungi and parasites.
Ⅱ.Medical Conditions:
Very rarely, sore throat is due to a medic- al condition whose etiology is poorly und- erstood. These conditions include Kawasaki disease, Stevens-Johnson disease, Beh-Cet’s syndrome, recurrent aphthous stomatitis, and systemic lupus erythematosus.
Ⅲ. Miscellaneous Causes:
Miscellaneous causes of sore throat include ①burns ②cancer ③leukemia ④irridiation ⑤referred pain from the thyroid ⑥chemotherapy ⑦polyps ⑧cigarette smoke is another miscellaneous cause. ⑨children in households with smokers often experience recurrent pharyngitis. ⑩Ingestion of irritating foods such as hot peppers, can irritate the intraoral tissue lining. ⑪ Inhalation of irritating chemical fumes or smoke from fires can also cause sore throat. ⑫Mouth-breathing results in xerostomia (dry mouth) and dry, sore throat.
